If you are a new patient, the forms below will need to be filled out when you arrive for your first appointment with Dr. Davis. Printing these forms ahead of time, filling them out, and bringing them with you will allow us to attend to your dental needs

more quickly than if you were to complete them upon your arrival, but it's not required!
